Aleph Practice Solutions was founded by pharmacists who spent years caring for skilled-nursing, hospice, and post-hospital patients, and kept seeing the same pattern: a fall, a hospitalization, a confusion episode, and upstream of it all, a preventable medication issue no one caught in time.

A dose misread. A warning printed in a language the patient doesn't read. An interaction nobody explained in words they could use. By the time it shows up as an ER visit, it stopped being a medication question weeks ago.

So we changed where we work: reach people earlier, at the moment they're actually holding the bottle. We build the infrastructure that catches the problem: at the counter, on the label, in the patient's own language.
